Amend a certificate: printed forms
After submitting a Medical Certificate for Cause of Death, … you can submit an amended certificate to replace the original.
To do this, you must:
- complete a new medical certificate in full
- sign and date it with the current date
- write ‘Amended Certificate’ at the top of the certificate.
You can return the amended certificate by:
A. attaching the document in an email to these two government departments:
- the Ministry of Health: email Mortality_Administration@moh.govt.nz
- the Department of Internal Affairs: email ISBDM.RegistrationsTeam@dia.govt.nz
B. by posting to:
- Mortality Administration, Ministry of Health
- PO Box 5013 Wellington 6145
The cause of death coding will be reviewed and updated if required; and official BDM Death certificates issued following the updates will contain the correct information.
The Ministry of Health and the Births, Deaths and Marriages (BDM) Registry teams will update their records with the changes.
