The certifying doctor or Nurse Practitioner must be readily identifiable
  • The full name of the practitioner should be recorded.
  • The full name should be the same as that registered with the Medical Council or Nursing Council of New Zealand: don’t use an unofficial shortened or “anglicized” version or a nickname.
  • “Qualifications” of the certifying practitioner refer to the academic degree that enabled the practitioner to gain a Practising Certificate. It does not refer to a job description such as “registrar” or “GP”.

All changes made on the certificates should be clearly initialed by the certifying practitioner.
