- Death in utero (i.e. foetal death before delivery)
- Gestation less than 20 weeks & weight less than 400 g

STILLBORN
- Death in utero (i.e. foetal death before delivery)
- Gestation more than 20 weeks or weight more than 400 g
Cremation certificate: not needed
NEONATAL DEATH
- Any delivery where the infant took a breath or had a pulse after delivery.
- Any death within 28 days of delivery
Cremation certificate: Doctor or Nurse Practitioner issues Cremation Form B and AB
NATURAL DEATH IN ANY PERSON OLDER THAN 28 DAYS
Death certificate: Doctor or Nurse Practitioner issues a Medical Certificate of Causes of Death (Form HP4720).
Cremation certificate: Doctor or Nurse Practitioner issues Cremation Form B and AB
CORONERS CASE
If the Coroner accepts a case, no doctor or nurse should issue any certificates
Death certificate: Coroner issues a Medical Certificate of Causes of Death
Cremation certificate: Coroner issues Cremation Form C and AB
