WHICH CERTIFICATES TO ISSUE?

MISCARRIAGE
  • Death in utero (i.e. foetal death before delivery)
  • Gestation less than 20 weeks & weight less than 400 g
Death certificate: not needed
Cremation certificate: not needed

STILLBORN
  • Death in utero (i.e. foetal death before delivery)
  • Gestation more than 20 weeks or weight more than 400 g
Death certificate: Doctor or Midwife issues Medical Certificate of Fetal & Neonatal Death (HP4721)
Cremation certificate: not needed

NEONATAL DEATH
  • Any delivery where the infant took a breath or had a pulse after delivery.
  • Any death within 28 days of delivery
Death certificate: Doctor or Midwife issues Medical Certificate of Fetal & Neonatal Death (HP4721)
Cremation certificate: Doctor or Nurse Practitioner issues Cremation Form B and AB

NATURAL DEATH IN ANY PERSON OLDER THAN 28 DAYS
Death certificate: Doctor or Nurse Practitioner issues a Medical Certificate of Causes of Death (Form HP4720).
Cremation certificate: Doctor or Nurse Practitioner issues Cremation Form B and AB

CORONERS CASE
If the Coroner accepts a case, no doctor or nurse should issue any certificates
Death certificate: Coroner issues a Medical Certificate of Causes of Death
Cremation certificate: Coroner issues Cremation Form C and AB
